Registration can be completed from 8 am until 12 pm. Please come to the AHS Office located at 430 Greenwood Ave, door 10.
If you are coming from outside of CMCSS, please bring the following information with you:
- Tennessee state-issued ID
- Birth certificate
- TN Immunization Record (you can get yours transferred from another state at the health department located at 330 Pageant Ln
- Two proofs of residency from the list below:
Mortgage bill
Property Tax Receipt
Current lease (within last 12 months) naming you as a tenant
Utility bill (electric, gas, water, waste disposal, cable)
Vehicle registration or current car insurance
Federal tax record, Social Security document, State assistance document
